If you bought butter or cheese directly from a National Milk Producers Federation Cooperatives Working Together Program member between December 6, 2008, and July 31, 2013, you could receive a payment. Class Members must submit a valid claim with or without documentation no later than April 23, 2021 to get payment. Claim Forms submitted or postmarked after this deadline will be deemed untimely and will not accepted. Eligible Class Members can submit one (1) of the following types of Claim Forms:

If you would like to claim purchases of butter and/or cheese made from Members of Defendant, Cooperatives Working Together and/or their subsidiaries from December 6, 2008 to July 31, 2013, and have documentation of said purchases, please complete and submit the "Submitted Documented Claim Form" with proof of your purchases. Documentation that is provided must be clean, readable copies. Originals will not be returned to you. Supporting documentation includes but is not limited to: Receipts, Invoices, Purchase Journals, Accounts Payable, Journals, and Bank Statements in addition to a receipt. Your supporting documentation must show: The date of the purchase, The name, address, and telephone number of the facility that made the purchase, Dollar amount purchased, Proof of payment including the amount paid for purchase and the date and manner of payment, Product description, and Quantity purchased. |
If you do not have documentation to show your butter and/or cheese purchases, please complete the "Undocumented Claim Form". The Undocumented Purchase Claim Form allows you to claim purchases of butter and cheese without providing documentation of those purchases such as store receipts showing you purchased butter and/or cheese directly from a settling defendant during the Class Period. The maximum payment for an Undocumented Purchase Claim is $5.00. If you are filing on behalf of the Claimant, you will be required to provide proof of authority to represent the Claimant in submitting the Claim. |
The Claims Administrator will review your submission and determine your payment. Undocumented Claims will be paid first. Payment of Documented Claims will follow.
Please Note: You should only submit one (1) Claim Form regardless if you have a mixture of documented and undocumented purchases.
All Claim Forms must be submitted online or postmarked no later than April 23, 2021.
